ok - I think I found more useful info by constructing a page as in the documentation. Testing in the Browser, seems to be broken in the way I described. Testing the app via the hybrid app (running on IOS) is working ok. Please verify you have tested via the Browser emulator? See screenshot attached ...

Any update on the custom icons?
I followed the recipe in the doc, but the icons do not show up in visual editor - therefore are not accessible for placement using the VE.
