How to put more than one column db csv.

Hello , I am Brazilian , so excuse me any grammatical error .
I wonder how could enter information more than two columns in the database , using an excel file in csv .
Also, how can I put these columns separately in the app.

For this data in your .csv file should be present correctly.
Can you show the first few lines (you can put dummy data) from the file you are trying to import?
