Even though I uploaded my own icon in the app prior to exporting the app it still shows the lime green appery icon on my iPhone for the app icon when I download it from iTunes. How can I change this to my own icon?

I have uploaded the 57x57 and 114x114. It will not let me add any other sizes. When I browse and upload an image it does not show no matter what I do.

I've downloaded to my iPhone 5 and iPad 3 both operating with OS 7.0.4
