I made my perfect app - but it has no functionality!

I think that I have created exactly what I want, which is an app for my business. It is a simple form that a client can fill out and have emailed to us. The app form is beautiful but I have no idea how to collect the data my client has input and have it sent to my business as an order. In addition, I would like to have a "call us" and "email us" button so a client can call us right from the app.
Even this simple step has me way over my head. I have researched similar problems and don't understand the answers I read. If I can make this work I'd like to upgrade to the paid platform and add some other apps I could use for this business, so I don't just want to be a "free" customer.
Help!
