How do I copy a form with Controls on it and give it a new name

I have a form with controls and want to make a copy of it and give it a new name.

Hello! Do you mean you want to copy page with controls? You can duplicate the screen by clicking on a gear icon on screen (but unfortunately there is a bug when duplicated screens can't be opened, we'll fix it next release). Another way is to create screen template.
