My create database service will not work!

I have created a simple form that ask the user to input some information. When the user click the submit button the create service suppose to invoke and in response it leaves the ID and createdAt data, but when I click the button the ajax spinner appears for a second and disappears. The information is never sent to the database. Can someone please help me? If needed I will share my project.

Have you debug your app - are there any errors in the console?
