Trouble with using .csv tables

So I am trying to build an app that gets its information from a complex table, where by selecting two different values will come up with a certain number (think of an excel spreed sheet, where the value A10 will come up with a different value that G3). I have uploaded the table to my database however, thats as far as my current knowledge can take me. Can someone please tell me (like I am a five year old) how do I get these values where they need to be.

Please specify following things:

1. On the excel screen shot - what should be passed to these two selects on the real app.

2. Screen shots of collections that you got after import processing.
