Custom Fields usins SendGrid API

I am using the SendGrid API to build a contact-form inside my app. I added some custom-fields, and I mapped them. The test runs OK, but the custom fields I add don't show up in the e-mails sent. Is it possible to edit the fields included in the e-mail?

Is it possible to create only these fields in a form? I'd like to do a custom form. Any other possibilities?
