Storing data from a form and adding a photo to it

Hello, I am trying to develop an app for historic architectural survey. The app will include a camera screen and a form screen. The form screen will include four or five short answer items, and I would also like to append a photo of the building described to the form. After the form is completed, I would like the information either stored in a local or cloud database or emailed to me. Any suggestions as to how to do this?
